Give the first result time to develop

An early result can look sparse without meaning the operation has failed. The NHS hair transplant guide places full results at around 10 to 18 months. Your surgeon should assess your progress before deciding that more grafts are needed.

New thinning and corrective work are different problems

Existing hair can continue to thin around transplanted hair. Someone who was pleased with their hairline several years ago may now notice a gap behind it. That does not, by itself, prove that the transplanted follicles have failed.

How much donor hair is left?

A previous graft count cannot tell the whole story. The surgeon needs to examine the hair that remains at the back and sides, including the pattern of earlier extractions, scars and any thinning.

Donor hair is limited. More grafts should not be taken just because an area at the front could look fuller. The likely improvement needs to justify the effect on the donor area and leave a realistic plan for future loss. Be clear about what a second procedure could change

Be specific about your priority. You might want better framing at the front, improved coverage through the middle or attention to the crown. Ask the surgeon to show which area could be treated and explain what would remain thin.

Also ask whether observation or treatment for continuing hair loss should come first. A second procedure has its own recovery, cost and surgical risks. If suitable donor hair is insufficient, declining further surgery may be the right recommendation.

Bring the story of your first procedure

Bring your previous treatment details if available: the procedure date, technique, graft count, operative report and photographs. Include a current medication list and explain any problems you experienced during recovery.
